Synovial Fluid
The viscous fluid produced by the synovial membrane inside joints. It lubricates joint surfaces, provides nutrients to articular cartilage, and contains hyaluronic acid for shock absorption.
Synovial fluid (also called synovia) is the clear, viscous fluid that fills the space within synovial joints. It is produced by the synovial membrane — the inner lining of the joint capsule — through a process of filtration from blood plasma combined with secretion of hyaluronic acid by specialized synoviocytes.
Functions
Synovial fluid serves three primary functions:
- Lubrication: the high concentration of hyaluronic acid and lubricin proteins creates extremely low-friction conditions between articular cartilage surfaces. Healthy joints have friction coefficients lower than ice on ice.
- Nutrient supply: since articular cartilage has no blood supply, all oxygen and nutrients reach chondrocytes by diffusion through synovial fluid. Joint loading during normal movement cycles fluid through the cartilage matrix, maintaining nutritional supply.
- Waste removal: metabolic byproducts from chondrocytes diffuse into synovial fluid and are cleared by the synovial membrane.
Changes in Disease
Synovial fluid analysis (arthrocentesis) is a key diagnostic tool in joint disease:
- Osteoarthritis: increased volume, decreased viscosity, mild increase in white cells (typically <5,000/µL)
- Septic arthritis: turbid fluid, markedly elevated white cells (>40,000/µL, predominantly neutrophils), may culture bacteria
- Immune-mediated polyarthritis: elevated white cells with sterile culture, consistent with immune-mediated rather than infectious cause
- Hemarthrosis: blood in fluid, seen with trauma or clotting disorders
Therapeutic Relevance
Injectable hyaluronic acid (HA) products are used in veterinary practice to supplement synovial fluid viscosity in arthritic joints. Evidence for efficacy is inconsistent — moderate improvement in some studies, no benefit in others. Oral HA supplements show limited bioavailability to joint spaces.
